Evidence-based practice. We hear these words often, but are we clear about what they really mean? If you are a newer nurse, you may not remember that at one time, only diabetic patients had blood glucose monitoring done routinely. Now, the importance of maintaining serum blood glucose levels within a normal range for all patients is well understood and appropriate steps for monitoring and treating altered levels are part of most hospital policies. That is evidence-based practice: using research findings to improve our practice.

When assessing for dehydration, remember the following signs and symptoms:

  • thirst
  • weight loss
  • dry mucous membranes
  • sunken-appearing eyes
  • decreased skin turgor
  • increased capillary refill time
  • hypotension and postural hypotension
  • tachycardia
  • weak and thready peripheral pulses
  • flat neck veins when in the supine position
  • oliguria
