Some staggering statistics are available regarding chronic disease. The numbers below are from the Centers for Disease Control and Prevention's Chronic Diseases and Health Promotion page (they have an extensive list with references, if you're interested in more information).

How can we, as nurses, support patients in the prevention and management of chronic illness? Patient education is essential as lack of physical activity, poor nutrition, tobacco use, and alcohol consumption are modifiable risk factors. The articles listed in More Resources are also good resources to help answer this question. Self-management, or self-care, is a common theme.
