In an effort to improve care and lower costs, in October of 2012, the Centers for Medicare and Medicaid Services (CMS) began applying reimbursement penalties to hospitals with excess readmissions for heart failure, acute myocardial infarction, and pneumonia. A nice summary of the details and issues associated with this program can be found in this Health Policy Brief: Medicare Hospital Readmissions Reduction Program, produced by Health Affairs through a grant from the Robert Wood Johnson Foundation.
