Best Practices for Heart Failure: A Focused Review

To provide safe and effective care for patients with heart failure, the nurse must recognize potential causes of exacerbation, understand them, and apply evidence-based practice guidelines aimed at reducing risk of exacerbation, carefully assess and monitor the patient, and provide effective patient and family education and self-management support. This article provides an overview of clinical, self-management support, and care transitions best practices for heart failure care in home care.
