Are hospital incident reporting systems flawed?

According to a new report from the Department of Health and Human Services’ Office of the Inspector General, hospital incident reporting systems captured only an estimated 14% of the patient harm events experienced by Medicare beneficiaries. Because hospitals rely on incident reporting systems to track and analyze events, improving the usefulness of these systems is critical to efforts to improve patient safety.

People who have heart attacks in the United States are far more likely to be readmitted to the hospital within 30 days than people in 16 other countries, a new study indicates.
