I'd like to add a few points to "Bloodborne Pathogen Exposure" (Action Stat, September 2004). The scenario involved a nurse injured by a self-contained needle/syringe after using it to administer a vaccine.

* The very first priority after a needle-stick injury is first aid.

* Rather than disposing of the sharp, I'd recommend sequestering it because it didn't self-contain. It could be a single faulty device, or the entire lot it came from could be faulty.

* If the nurse's titer to hepatitis B virus is low, he should receive hepatitis B immunoglobulin.

* According to the most recent bloodborne pathogens standard, employees must be asked for their recommendations for preventing similar situations.
