Heart failure accounts for a vast number of hospital admissions and rehospitalizations among older adults. The complexity of this condition necessitates an evidence-based, patient-focused, interdisciplinary approach to care. This article presents the efforts of clinical nurse specialists, a nurse manager, and nursing staff on a progressive cardiac care unit to develop, implement, and evaluate a pilot evidence-based group discharge education program for patients with heart failure and their families. The journey taken to launch this initiative is described, and program outcomes are presented. Major emphasis throughout the project was placed on promoting patients' self-management of their condition and further integrating evidence-based practice within the organization.
