ASMBS: Gastric Bypass Tied to Improved Conception Rate

Excellent postoperative conception rates, symptom relief achieved in obese women with PCOS

WEDNESDAY, June 15 (HealthDay News) -- In morbidly obese women with polycystic ovarian syndrome (PCOS), weight loss after Roux-en-Y gastric bypass (RYGB) achieves amelioration of PCOS with improved postoperative conception rate, according to a study presented at the annual meeting of the American Society for Metabolic and Bariatric Surgery, held from June 12 to 17 in Orlando, Fla.

Mohammad K. Jamal, M.D., from the University of Iowa Hospitals and Clinics in Iowa City, investigated the long-term effects of RYGB on PCOS and infertility in 20 morbidly obese women with PCOS who underwent RYGB surgery between 2000 and 2009. The women had a history of obesity and PCOS for an average duration of 20 and 8.6 years, respectively, were an average age of 32 years, and had an average body mass index of 52.8 kg/m². Oligomenorrhea, hirsutism, and type 2 diabetes mellitus (T2DM) were present in 85, 70, and 45 percent of the cases, respectively. Ten patients did not conceive prior to surgery. The participants were followed up for an average of 46.7 months.

The investigators found that an average of eight months following RYGB, menstrual cycles became regular in 82 percent of cases, complete remission of T2DM occurred in 77.8 percent of the cases, and 29 percent of the patients with hirsutism had symptomatic relief. Of the patients who did not conceive before surgery, six conceived within three years after surgery (five spontaneously and one with in-utero insemination), while four did not desire pregnancy.

"Surgical weight loss following RYGB achieves excellent amelioration of PCOS manifestations, and the postoperative conception rate in infertile PCOS subjects desiring pregnancy was 100 percent," the authors write.
