AN ADMISSION ASSESSMENT is the best way to identify patients at risk for pressure ulcers. But your admission assessment is valid only if you gather all the information you need.
At our facility, nurses once completed only about 30% of risk assessment forms for pressure ulcers and falls. After the form was redesigned and streamlined, the completion rate rose to 88%and patient care improved as a consequence. What worked for us may work at your facility too.
One problem with the old form was that it was separate from the primary admission form, which may have encouraged nurses to regard it as secondary in importance. It also called for information already gathered on the primary form, and content was grouped according to the services that were available instead of what the patient needed.
