HEART FAILURE (HF) is the primary hospital admitting diagnosis for people over age 65. Developing strategies to monitor a patient's condition and response to treatment is essential to improve outcomes.
At my facility, we're committed to providing all patients with HF the best available care. To ensure this, we've formed a multidisciplinary HF subgroup. In this article, I'll describe how it's improved care for patients with HF.
Tracking core measures of HF
In 2001, the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) published several quality initiatives for HF and identified certain core measures that are essential in treating HF. In 2005, JCAHO updated these guidelines. The mission of the HF subgroup is to ensure consistent care for patients with HF in compliance with these JCAHO measure sets for HF care.
