Abstract

SUPPOSE YOU DISCOVER that a colleague is falsifying the medical record; for example, by documenting medications that weren't given or by noting that a patient ate a full meal when she didn't. You're responsible for reporting incompetent, unethical, unsafe, or illegal practices—including falsified documentation. Not doing so could jeopardize your license.
