In 2006 the Institute of Medicine issued a report, From Cancer Patient to Cancer Survivor: Lost in Transition, which recommends that every cancer patient receive an individualized survivorship care plan that includes guidelines for monitoring and maintaining their health. In response to that report and following the recommendations put forth at an AJN-sponsored conference in 2005 on nursing's role in survivorship, a national group of nursing, social work, and survivor organizations, and other key stakeholders met in November 2006, to develop a Master Plan for improving care for cancer survivors. They concluded that while many disciplines should collaborate in this initiative, the development of this “prescription for living” and its updating should begin with nurses. The November stakeholder meeting was held in collaboration with the American Cancer Society, the Oncology Nursing Society and the National Coalition for Cancer Survivorship. Partial funding was provided by the American Cancer Society and the Oncology Nursing Society.
