    IN October 2002, the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) announced “Shared Visions—New Pathways,” the next phase in the evolution of health care organization accreditation survey process, in response to the changing needs of populations served and changes in the healthcare industry. JCAHO believes that by shifting focus of the accreditation process to examining the interrelated processes of patient handoffs (eg, internal transfers of patients between units, departments, and clinical services) and an organization's continuous improvement activities of these hand-offs, and to assure safe care delivery practices, organizations will eliminate activities solely geared to survey preparation and last-minute compliance with standards.

    Effective care delivery practices and safer patient care are the desired outcomes of changes in the accreditation process focusing on

    * Actual delivery of clinical care;

    * Increasing value and satisfaction of the accreditation process to accredited organizations, the public, and staff;

    * Decreasing costs/activity related to survey ramp-up/readiness activities (eg, looking for patient records that are compliant with JCAHO documentation standards, and correcting systems and processes that should have been fixed long before the actual survey); and

    * Increasing confidence of the public that health care organizations continuously comply with standards that are in place to promote safety and quality, and not just compliance to written documents.
