Joy Ufema pioneered the role of death-and-dying specialist at Harrisburg (Pa.) Hospital in 1973. She's currently clinical specialist at Upper Chesapeake Medical Center in Bel Air, Md., and at Harford Memorial Hospital in Havre de Grace, Md.

My neighbor, “Ann,” is a divorced 46-year-old woman whose twin daughters are in my son's fourth-grade class. At times she's asked me for advice about minor childhood illnesses .

Another neighbor just told me that Ann is being treated for terminal breast cancer. Apparently she refuses to discuss her condition or prognosis with anyone, including her minister or even members of the oncology staff .
