I'm a nurse practitioner working in a family practice. Recently, a patient who's adopting a child from Korea asked me to write a letter verifying her health status. Because she's 32 years old and in good health, I had no problem with her request—until she asked me not to mention that she's a lesbian. She's living with a committed partner and has been quite open with me about her sexual orientation, so I asked why. She said she's afraid that the adoption agency will drop her application if her sexual orientation is revealed .

I want to support my patient, but I'm uncomfortable with the deception. What's my obligation here ?—H.G., S.C.
