  • QUESTION: I'm caring for an older adult with cancer who has a mix of chronic and acute pain: chemotherapy-related neuropathy in her feet, low back pain from spinal metastases, and abdominal pain from recent surgery. Although her overall condition is stable, she rates her pain as 8 on a 0-to-10 scale. Her drug regimen includes a 50-mcg transdermal fentanyl patch, morphine elixir for breakthrough pain (15 mg every 4 hours as needed), and 300 mg of gabapentin taken twice a day.
