JOHN PEARSON, 42, suffered second- and third-degree burns in a house fire when he was 8 years old, and has extensive scarring down both arms, his neck, and the left side of his face. Three months ago, he noticed a painless lump in scar tissue on the medial aspect of his upper left arm. The lump slowly eroded into a bleeding ulcer that now measures 4.2×2.8 cm (1.7×1.1 inches).

A biopsy of the ulcer reveals squamous cell carcinoma, and Mr. Pearson is diagnosed with Marjolin's ulcer.

What is Marjolin's ulcer?

Named after the French surgeon who first described it, Marjolin's ulcer is a malignant degeneration arising in a chronic wound or scar tissue. Besides burn scars, Marjolin's ulcers can develop in chronic pressure ulcers, frostbitten tissue, skin graft donor sites, lupus rashes, neuropathic ulcers, sinus tracts, urinary fistulas, irradiated skin, and vaccination sites.
