WRONG-SITE SURGERY made national headlines in 1995 when a man had the wrong leg amputated in a Florida hospital. The tragedy focused national attention on this serious issue.

As the case was analyzed, information surfaced that helped frame this case of wrong-site surgery as a systems problem rather than simply an individual performance error. In this article, I'll spell out measures to improve both systems issues and individual performance to enhance patient safety. Let's start by defining some key terms.
