ABSTRACT
Emergency asthma treatment

▪ A new report indicates that adults with acute severe asthma receive an additional benefit from multiple doses of inhaled albuterol, ipratropium bromide, and flunisolide. Researchers conducted a randomized, double-blind, prospective trial. Out of the 172 patients who presented to an emergency department for acute severe asthma, 56 received the triple-drug treatment, while the rest received one of two two-drug combinations. Patients receiving the three-drug combination had a 64% greater improvement than those who received either of the two-drug combinations.

According to the study, patients who'd most benefit from the three-drug treatment were those with more severe obstruction. In addition, the three-drug combination affected hospital admission rates. Three hours after treatment, only 11% of the triple-drug treatment group was admitted to the hospital, as opposed to 25% and 20% of the two-drug treatments, respectively.

    ▪ The American Heart Association supports the use of automated external defibrillators (AEDs) in children, according to a recent statement released by the organization. After conducting a literature review, the authors of the statement approved use of AEDs for children between the ages of 1 and 8 who have no sign of circulation. The statement indicates that ideally, the device should administer a pediatric dose, and rescuers should attempt 1 minute of CPR before using an AED.

Prior to the release of this statement, a child younger than 8 years old could receive manual defibrillation at a hospital, but not with home defibrillators used by emergency personnel or laymen. The statement encourages manufacturers to continue developing pediatric rhythm diagnostic programs.
