Marian Soat is a clinical nurse specialist in cardiology at the Cleveland Clinic, Cleveland, Ohio.

More than 1 million people with heart failure are admitted to U.S. hospitals each year, many of them with fluid overload. Loop diuretics are usually administered to reduce this fluid overload and relieve respiratory symptoms. But for many patients, diuretics become less effective over time. Additionally, because long-term studies on the effects of loop diuretics in heart failure patients are lacking, mortality and morbidity rates are unknown.

Ultrafiltration—the removal of fluid via an intravenous (I.V.) line—may be a safe and effective alternative for the heart failure patient who's responding poorly to loop diuretic therapy. Because it's relatively easy to implement and maintains patient stability, ultrafiltration can be used in the step-down unit.
