This issue highlights a very exciting collaboration between the journal and the Agency for Health Care Research and Quality. Shepherded by Associate Editor John Wasson and organized by Rebecca Roper together with the assistance of Dr Wasson, the issue focused on primary care research networks. Not only does this issue highlight exciting new research findings, but it does so in a critical area-primary care-that needs to witness dramatic advances if health reform is to systematically succeed.

This issue appropriately highlights a new regular feature of the journal-commentary by Rushika Fernandopulle, MD. Founder of Iora Health, Dr Fernandopulle believes that for health reform to succeed, primary care needs to be completely redesigned from the bottom up. He will be offering his observations from his front line on a quarterly basis. I welcome similar "snapshot" from fellow readers.

Finally and irreverently, we conclude with observations from the front lines of Texas health reform.
