Abstract
The nurse's ability to provide accurate baseline and ongoing assessment is particularly crucial for cardiac patients transitioning from acute care to home care. This article examines the guidelines for cardiac home care patients, and discusses the importance of and techniques for effective assessment of patient history, function, and cardiac and fluid status.
