The Diabetes-Cardiovascular Disease Tool Kit was developed by a collaboration of the American Diabetes Association, the American College of Cardiology, and the PCNA to help you educate your diabetic patients about their high risk for CVD. This tool kit contains information about 25 topic areas that may affect your patient, including type 2 diabetes and glucose control, risk factors and lifestyle management, coronary heart disease, and vascular disease. In addition, this tool kit includes forms for tracking key goals and logging daily eating and physical activity patterns.

 

Why is it important to educate diabetic patients about the risk of CVD?

 

* Cardiovascular disease is the leading cause of premature death among people with diabetes, yet people with diabetes remain largely unaware that they are at such a great risk.

 

* As a healthcare professional, you can help your patients Make the Link between diabetes and CVD. Help them understand that diabetes is not only managing blood glucose, but managing blood pressure, cholesterol, and other CVD factors as well.
