Abstract

Effective transitioning of clients with chronic illnesses from the hospital to home care has been successful in improving quality of life, requiring fewer readmissions, and reducing costs. When these clients become ineligible for covered skilled care, what cost-effective strategies can be used to assure the positive outcomes achieved will continue? Based on an evaluation study, this article outlines how volunteer mentors can assist patients to remain safe in their homes.
