NCPD TEST QUESTIONS

LEARNING OUTCOME: Participants will demonstrate knowledge of issues related to patients undergoing total knee arthoplasties (TKAs) in the ambulatory surgery center (ASC) setting by achieving a minimum score of 70% on the posttest.

LEARNING OBJECTIVES: After completing this continuing professional development activity, the participant will be able to:

1. Discern criteria for preoperative screening of patients for TKA in the ASC setting.

2. Identify potential complications of TKA and measures to mitigate those complications.

1. In the 2018 study by Nussenbaum et al., preoperative screening criteria for patients undergoing TKA included

a. a hemoglobin >= 13.

b. an albumin level >= 4.0.

c. a hemoglobin A1c <= 7

2. After implementing the preoperative screening, which post-operative complication decreased from 4.4% to 1.3% for TKA and total hip arthroplasty patients in the Nussenbaum et al. (2018) study?

a. surgical site infection

b. pneumonia

c. venous thromboembolism

3. Although the optimal age for TKA in the ASC setting is unknown, Krause et al. (2018) suggested that most patients who are ideal candidates for having TKA in the ASC setting are under the maximum age of

a. 55 years.

b. 60 years.

c. 65 years.

4. According to Krause et al. (2018), a preexisting health-related condition that should exclude patients from having TKA in the ASC setting is

a. significant visual impairment.

b. chronic opioid consumption.

c. history of previous post-operative complication(s).

5. Patients classified as American Society of Anesthesiologists (ASA) class III are generally considered to be high risk for perioperative complications. What body mass index (BMI) would classify a patient as ASA class III?

a. >= 35 kg/m2

b. >= 37.5 kg/m2

c. >= 40 kg/m2

6. When considering the ASC setting for elective TKA, the authors report that a patient with a BMI classified as ASA Class III is at increased risk for

a. adverse effects from medications.

b. increased pain.

c. decreased ambulation potential.

7. Joshi et al. (2012) suggested that patients with known obstructive sleep apnea may be considered for surgery in the ASC setting only if comorbid conditions are optimized and if, after discharge, the patient is able to

a. use a continuous positive airway pressure device.

b. have an adjustable "hospital" bed available.

c. use oxygen supplementation at home.

8. Patient education related to post-operative complications following TKA should incorporate information about prevention of the most common complications, including

a. falls.

b. opioid dependence.

c. depression.

9. The authors note that common evidence-based findings often incorporated into patient self-management postoperative pain protocols include the use of

a. acetaminophen.

b. nonsteroidal anti-inflammatory drugs.

c. acetylsalicylic acid.

10. Wittig-Wells et al. (2015) reported an intervention that may be more likely to decrease pain after TKA is

a. single, 30-minute applications of cryotherapy.

b. alternating applications of cryotherapy and heat therapy.

c. consistent, sequential applications of cryotherapy.
