The article "Caring for a Patient after Coronary Artery Bypass Graft Surgery" (March 2008)* is very interesting and informative. However, as a home health care nurse, I was disappointed that you didn't recommend evaluating the patient for short-term home health care follow-up as part of his discharge planning.

During the healing period, the home health care nurse can continue to assess, teach, and reassure patients and families before cardiac rehabilitation begins. Additionally, visiting the patient in his home affords a unique perspective that can't be replicated anywhere else. A home health care nurse can use clues in the home to identify needs and strengths and tailor the care plan accordingly.
