Abstract

Below you will find summaries of four published articles that describe recently published research on chronic disease care coordination. The first describes chronic disease patient experiences with healthcare systems caring for patients from eight countries. Readers will note that there is significant room for improvement, particularly in the United States. The second summary describes a recent care model developed to improve coordination of care for multimorbid older patients that is currently under study. The third article describes the successful implementation of a reengineered hospital discharge program. The final summary provides findings from a meta-analysis of heart failure care management programs and reports on a couple key components to a successful implementation.
