GUIDELINES:
* Members of the NBCHPN(R) Board of Directors and members of the Awards Subcommittee are ineligible to receive these awards.
* All nominations (for both CHPN(R) and CHPNA of the Year) are reviewed, and the recipient is selected by the Awards Subcommittee of the NBCHPN(R).
* Recipients are responsible for travel costs and related expenses.
SUBMISSION REQUIREMENTS (for both CHPN(R) and CHPNA of the Year):
* Completion of Nomination Form (download from Web site at http://www.nbchpn.org or call the National Office at 412-787-1057).
* Documentation of how nominee meets criteria for this award and further elaboration on the individual's qualifications for this award (eg, relevant employment, involvement in special initiatives related to end-of-life nursing care). This documentation should be no longer than 2 pages (8 x 11)
* Up to two (2) letters of support from others in the field who can provide validation of nominee's contribution to end-of-life nursing.
All materials must be received in the National Office by November 30, 2004. Faxed or e-mailed materials will not be accepted.

NBCHPN(R) Hosts Fourth Annual Certification Recognition Event
NBCHPN(R) will host its Fourth Annual Certification Recognition Event on Saturday, January 22, 2005, from 11:45 AM to 1:00 PM at the AAHPM/HPNA Annual Assembly, Hilton New Orleans Riverside, New Orleans, LA. When registering for the Annual Assembly, please indicate your credentials. Reservations will be required to attend this event.
