Written documentation of care provided to GI patients is an integral part of an endoscopy nurse's daily routine. The nurses' note must capture the specifics of the patient's condition preprocedure, intraprocedure, and post procedure, as well as all medications given and procedures performed. It must be accurate, comprehensive, and factual. This presentation will provide a retrospective look at where we have come from and the progress we have made related to documentation. New JCAHO recommendations will be discussed along with tips to protect yourself from a lawsuit by documenting properly.
