A 68-year-old Caucasian male was referred to the dermatology clinic for a 6-week history of an asymptomatic rash on the thigh followed 4 weeks later by multiple similar-appearing lesions on the torso and extremities. Physical examination showed scaly erythematous plaques with a fine collarette of scale on the abdomen, extremities, and palms and thick scaly plaques on scalp, elbows, and knees. KOH preparation of the scales was negative. Significant history includes diabetes, psoriasis (scalp, elbows, and knees), allergic contact dermatitis, neurodermatitis, and prurigo nodularis.
